NDA Admissions Requirements for BHA Students
NDA Admissions Requirements for BHA students:
- Completed Inquiry Form - online*
- Birth Certificate
- Passport, if applicable
- Official copies of school transcripts, last 3 years
- Recommendation from a current teacher, principal and/or school counselor
- Copy of IEP/504 Plan, if applicable
- Interview with the NDA Director of Admissions
- Proof of all Massachusetts required immunizations or proof that an appointment to receive the required immunizations has been scheduled
NOTE: International students who need immunizations or away students who do not have a Primary Care Physician (PCP) in Massachusetts, may schedule an appointment at the Pediatric Immunization Clinic at Lowell General Hospital (978-937-6023). Students will need to a letter from the NDA School Nurse stating what vaccines are needed.
The Clinic Hours:
Tuesdays - 2:00 PM - 6:00 PM
Saturdays - 8:00 AM - 12:00 PM
